Job Description
The Community Health Worker (CHW) will support the MobileMoc program under the ROAD MAP Grant. The role focuses on delivering case management and social service support to elder adults and caregivers in the community, with additional duties in administrative support and public health programming. The CHW will operate withing the defined scope of practice, MobileMOC policies, and grant guidelines.


Responsibilities
Case Management & Resource Navigation
  • Conduct health and social service assessments for older adults and caregivers
  • Make referrals, provide follow-up, and connect clients with community resources
  • Build and maintain partnerships with organizations serving target populations
  • Train MobileMOC staff on navigating health and social service resource

Program Development & Implementation
  • Assist in community needs assessments and data collection
  • Help design, distribute, and analyze survey tools
  • Support the planning and delivery of community-based health programs

Administrative Support
  • Provide registration and logistical assistance for MobileMOC clinics and events
  • Participate in community outreach events, such as health fairs

Ideal Candidate Profile:
  • Knowledge of social services, community resources, and referral processes
  • Strong observational and needs assessment skills
  • Ability to manage electronic health records (EHR)
  • Professional and effective communication skills with students, faculty, community partners, and staff

This position is a Limited Duration Appointment (LDA).

A Limited Duration Appointment (LDA) is a form of regular employment for a specific project or for a limited period. An LDA may be established for up to one year with an option to renew the appointment annually for a maximum of three years. Please note that this position may be converted to a permanent position prior to or at the conclusion of three years, funding dependent.

Qualifications
Minimum Qualifications

High school diploma or GED and two years of relevant experience, or an equivalent combination of education, training, and experience.

Preferred Qualifications

One (1) or more years of experience as a Community Health Worker or Medical Assistant; certification as a Community Health Worker or Medical Assistant; BLS/CPR Certification for Healthcare Providers; experience in community or public health settings; and bilingual abilities.
